Intensive Consent Form

Thank you for signing up for the seminar. We look forward to working with you. Please review the following information, and sign at the end to acknowledge your agreement with what you have read.

Confidentiality: Confidentiality is of paramount importance. The agreement among all seminar
participants to strictly maintain confidentiality is part of what makes the deep work of the seminar possible. It creates the safety necessary for people to open up and develop the bonds of trust between members that is an essential part of the experience. By signing at the bottom, you agree to keep everything you learn about others in this seminar completely confidential, as they
will agree to do for you.

Coaching: This is not a seminar in psychotherapy but rather a coaching experience. That means that Dr. Pressman is not functioning as a psychotherapist and cannot provide the same services as one would expect from a psychotherapist. Specifically, this seminar focuses on the Deconstructing Anxiety method and, as such, follows a set of principles and practices that are different from those offered in psychotherapy. Dr. Pressman functions as a coach, a facilitator, of this method, as opposed to a therapist utilizing the methods of standardized psychotherapeutic schools of thought.

Availability: Dr. Pressman is not available for communications before, after or in between group
sessions.

Insurance: Since this is not a therapy experience, no insurance reimbursement is available.

Responsible for self: If you are having a mental health crisis, you agree to find appropriate help,
and understand that Dr. Pressman cannot be available for contact in such situations. During the
seminar, it is understood that you are responsible for your own self-care, and that neither Dr. Pressman nor anyone on his staff can function as a source of help or support for circumstances beyond the focus of the seminar (the principles and practices of the Deconstructing Anxiety method).

No-harm contract: You agree unequivocally that you will not harm yourself during the time of our work together. You further agree that should you have thoughts about self-harm, you will discuss them in session. Finally, you understand, as stated above, that I am functioning as a life coach in these sessions and not as a psychotherapist, so I am therefore unable to provide the support and other services that a psychotherapist would in the event that you had thoughts of self-harm.

If for any reason Dr. Pressman feels the seminar is no longer the appropriate place for you to meet your goals, he reserves the right to conclude your participation in the program at any time. Of course, this would be done in discussion with you, and Dr. Pressman can suggest alternative options he believes would be more beneficial for you if you’d like. You would be entitled to a full refund of your tuition for the seminar under these circumstances.

Thank you for filling out this screening form. It is required for anyone who wishes to participate in a program offered by Pressman Seminars. All information on this form will be kept strictly confidential. It is important that you answer all questions thoroughly and honestly. We reserve
the right to deny admission to anyone we deem may not be appropriate for participation in a particular program. The purpose of this form is to help ensure that the program you wish to enroll in is the right place for you, and that Pressman Seminars is able to meet your needs effectively.

Are you a therapist? (Note: Being a therapist is not a pre-requisite for participation. All programs at Pressman Seminars have been developed for adults 18 years of age and older, whether a therapist or not).
